How to write it

  • Give both dates in full, in words and figures, and say which document carries each. Ambiguity about a date is the whole problem you are trying to fix.
  • Cite the birth certificate as the authority. A date of birth is settled by the birth registration, not by the school record or by family memory.
  • Say when you discovered the error and why it was not noticed earlier.
  • Acknowledge that the correction must go to the board as well as to the school register.

With due respect, I beg to state that I am a student of Class IX, Section A, of your school, bearing roll number 18. My date of birth has been entered in the school register as 3 March 2009 (the third of March, two thousand and nine), whereas my correct date of birth, as recorded on my birth certificate, is 13 March 2009 (the thirteenth of March, two thousand and nine).

It appears that the figure was miswritten when my admission form was copied into the register. I discovered the error only last week, when my registration card was distributed and I compared it with my birth certificate for the first time. I am enclosing a photocopy of my birth registration certificate, which is the authoritative record, and can produce the original at any time.

I, therefore, pray and hope that you would be kind enough to correct my date of birth in the school records and to forward the correction to the education board before my registration is completed, and oblige thereby.
