Why write the dates in both figures and words?

Because the error you are reporting is usually a misread or mistyped figure, and repeating each date in words removes any chance of the same thing happening again in the correction itself. It is a small precaution that makes the application unambiguous.

Which document settles a date of birth?

The birth registration certificate. School records, family memory and older documents are not authoritative, so the application should cite the birth certificate explicitly and attach a copy. Naming the right authority is what makes the request actionable.

Is this the same as a name correction application?

The structure is the same but the evidence differs, and so does the urgency: a wrong date of birth affects eligibility for examinations, jobs and passports for the rest of a person's life. The application should say plainly that the correction must reach the board before registration closes.

It appears that the figure was miswritten when my admission form was copied into the register.
