How do I report illness without diagnosing it?

Report what was observed and who observed it: several students absent with stomach complaints in a particular week, and a teacher noticing that most had drunk from the tap. That is an observation. Claiming the water caused a specific disease is a diagnosis you cannot support.

Should I offer more than one solution?

Yes, where a cheaper one exists. Asking for a filter but noting that repairing an existing tube well would also serve gives the school an inexpensive option, and an application that offers a cheap alternative is much more likely to be acted on.

Where should I ask for it to be placed?

Somewhere central enough to serve every class — an assembly ground or a main corridor. Naming the place shows you have thought about use rather than only about provision, and it saves a decision the school would otherwise have to make.

How to write it

Say what the present arrangement is and what is wrong with it. A request for water in a school that already has taps must explain why they are not enough.
Give the health consequence if there is one — students falling ill, absences after a particular period — but do not diagnose. Report what has been observed.
Ask for something specific: a filter, a deep tube well, a water cooler, or the repair of an existing one.
Note where it should be placed and roughly how many students would use it.

With due respect, we the students of your school beg to state that there is at present no safe drinking water on the school premises. The single tap in the courtyard is connected directly to the supply line, and the water from it is often muddy after rain and has an unpleasant smell. Most students therefore bring water from home, and those who cannot drink from the tap or go without.

Several students of our class were absent with stomach complaints during the last week of August, and our class teacher observed that most of them had been drinking from the tap. We request that a water filter be installed near the assembly ground, where it would serve all the classes, or alternatively that the existing tube well behind the science building be repaired, since we understand it has been out of order for two years only for want of a new hand pump.

We, therefore, pray and hope that you would be kind enough to arrange pure drinking water for the students and oblige thereby.
