How to write it
- Say plainly how the original was lost or damaged, and when. A duplicate is issued on the strength of that account, so it must be specific.
- Mention any report you have made — a general diary entry at the police station is usually required for a lost certificate.
- Give the examination, year, board, roll and registration numbers, exactly as in a correction application.
- Say why you need the duplicate now: an admission, a job application, a passport.

With due respect, I beg to state that I passed the Secondary School Certificate examination from your school in 2024 under the Rajshahi Education Board, bearing roll number 132458 and registration number 1811034521.
On 28 August this year our house was flooded when the embankment at Sujanagar gave way, and the almirah in which my documents were kept remained under water for two days. My SSC certificate and mark sheet were destroyed beyond use, and I am enclosing photographs of the damaged documents. I have also lodged a general diary at Sujanagar police station, and a copy of the entry is enclosed. I now require a duplicate certificate urgently, as I have been called for admission to a diploma course and the certificate must be produced by 15 October.
I, therefore, pray and hope that you would be kind enough to forward my application to the board for the issue of a duplicate certificate and oblige thereby.
